PERSONAL HEALTH AND MEDICAL RECORDS

Annual Health Form
All activities require a health examination within the past 12 months by a licensed health-care practitioner. This includes youth and adult members participating in all troop activities. Save a copy of this form on your computer as you can type a lot of the information and save it for your records.

This new form replaces all the previous Class 1, 2, and 3 health forms and will have to completed prior to any long term camping.
